For incoming 7^th grade shots:

New requirements for 7^th grade immunizations are:

1.        The second varicella (if they haven't had the chickenpox disease).  If they have had the illness, we need the date.

2.       The MCV4, for meningitis prevention.

3.       Tdap, this is a tetanus booster with diphtheria and pertussis (whooping cough) vaccines as well.  If your student has had a tetanus booster within the past 5 years, they don't have to take this now.

You need to complete the following forms if you want your student to receive these immunizations at the shot clinic at registration on May 1st.

1.       Texas Vaccine for Children Information

2.       Immtrac consent form

3.       Chickenpox Addendum

4.       Meningococcal Addendum

5.       Td/Tdap Addendum

If you have these forms completed and bring them with your child to registration, it will speed up the process.  The shots are $5 each and a parent must be present (unless the forms are all completed and you send the money with your student).
